Minor Participation Waiver & Parental/Guardian Consent Addendum

Required for participants under the age of eighteen (18) at the time of the Promotion.

This Addendum is intended to supplement and be incorporated into the Release and Waiver of Liability, Assumption of Risk, and Indemnity Agreement (“Agreement”) issued by BTS Athletics. By signing this Addendum, I, the undersigned parent or legal guardian, affirm and agree that all terms, conditions, waivers, and releases described in the original Agreement apply fully to the Minor Participant named below. This Addendum does not replace the Agreement but extends its coverage to minors with proper parental or legal guardian consent.

Parental/Guardian Acknowledgment

I, the undersigned, am the parent or legal guardian of the minor listed below (the “Minor Participant”). I understand and acknowledge the following:

1. Consent to Participate: I authorize the Minor Participant to voluntarily participate in the programs, events, and promotions organized by BTS Athletics (the “Promotion”).

2. Agreement Extension: I acknowledge that I have read and fully understand the “Agreement” and agree that it shall apply in full to the Minor Participant. I voluntarily accept all risks identified therein on behalf of the Minor Participant.

3. Assumption of Risk & Release: I accept full responsibility for any and all risks involved with the Minor Participant’s participation and, on behalf of the Minor, release and hold harmless BTS Athletics, its owners, employees, agents, contractors, volunteers, sponsors, and affiliates to the fullest extent permitted by law.

4. Medical Authorization: I authorize BTS Athletics personnel or their designees to obtain medical treatment for the Minor Participant if needed in an emergency. I understand every reasonable effort will be made to contact me prior to any treatment.

5. Photo & Media Release: I grant permission for BTS Athletics to use photographs, video, or likenesses of the Minor Participant in marketing and promotional materials.

6. Proof of Age Requirement: I understand that valid proof of age may be required for participation and must be presented upon request.
